AFFIDAVIT OF CHANGE OF NAME

          I, Raymond Benjamin Chukwunonso, Male, 19 years, Christian, Student, a Nigerian Citizen living at No. 62, Matthew Street, Ado-Ekiti, Ekiti State do hereby make an oath and state as follows;

1.     That I am the declarant herein.

2.     That from my birth till sometimes within the year, 2007, the    full order of my name starting with the surname has been Onwuasoeze, Benjamin Chukwunonso.

3.     That sequel to my father’s change of surname to wit; from Onwuasoeze, Raymond Ngborie to Orji Raymond Mgborie, I deemed it fit to consequentially adopt my father’s first name (Raymond), as my surname.

4.     That all my former documents and references bearing Onwuasoeze as a Surname as against my present surname, Raymond, remains valid and should be treated as same; Onwuasoeze Benjamin Chukwunonso and Raymond Benjamin Chukwunonso being one and same person.

5.     That I make this solemn declaration, conscientiously believing same to be true to the best of my knowledge and by virtue of the provisions of the Oaths Act 1990.
