AFFIDAVIT OF LOSS OF ORIGINAL DOCUMENT

I, BRIGHT NWACHUKWU, male, adult, Christian, Nigerian citizen of No: 32 Ekwema Crescent, Owerri, Imo State do hereby make this oath and states as follows:

1. That my name is Bright Nwachukwu of the above named address and a driver staff of Kilimanjaro Fast Food, Owerri.

2. That on the 23rd day of May, 2015, I was booked for a sum of Sixty Thousand Naira, by the Federal Road Safety Commission over an alleged offence of dangerous driving while on transit. 

3. That accordingly, offence ticket number T0029500720 embodying the said amount was given to me to make payment with and I did a photocopy of the ticket. 

4. That I have since made the necessary payment required as regards to the alleged offence, but however cannot account for the existence of the original FRSC Offence Ticket aforesaid.

5. That save the photocopy of the document with me, I have made reasonable efforts to search for the said Original document and all my efforts proved abortive.

6. That I make this affidavit in good faith, for records purposes conscientiously believing same to be true and correct to the best of my knowledge and in accordance to the Oaths Act.
